Patient Information

 

New patients:We welcome new patients and families to our clinic.

 

Appointment:Please make appointment in person or call our medical centre to make an appointment. 

Please inform the receptionist when booking appointment if a longer time with your doctor is required.

We prefer patients make appointments but walk ins are also welcome.

Emergency cases are triaged by our staff and will be seen as a priority.

 

Online appointment is available via Health engine. Online appointments accepted.

You can walk in if unable to make an appointment and one of our Doctors will attend to you as soon as possible.

 

Opening hours:Our clinic is open 6 days a week.

Languages: English, Spanish, Afrikaans,Macedonia,Vietnamese,Burmese,Croatia,Arabic etc

The clinic also utilises the services of Translating and Interpreting Service which is available on 131450.

Please inform our friendly staff if you require an interpreter.

 

Home Visits are available during hours for those who cannot attend the surgery due to their medical condition/immobility/disability. Patients are encouraged to attend the surgery if they can with the assistance of their relatives or carers. If agreed that a home visit will have to be made, visiting time will be organised on the day according to the availability of doctors. Our Doctors will make home visits if absolutely necessary for clinic patients who cannot attend the clinic.

 

Personal health information:

We are committed to maintaining the confidentiality of your personal health information. Your medical record is a confidential document.

Our practice policy is to maintain security of your personal health information at all times and to ensure that this information is only available to authorised staff members.

No informaton contained in your medical record is to be released or transfered to a third party, such as another practice or an insurance company unless a request is made in writing by you.Our Doctors are not able to discuss personal information via email or text. Please do not send sensitive, confidential or electronic communication that requires urgent response via email. 

 

Scripts/Referrals/Medical certificates:Please book an appointment to see a doctor for any of these requirements

 

Pathology Test Results: Our staff DO NOT give results over the phone. If your results come back and your doctor needs to see you, our staff will call and arrange an appointment for you.Alternatively,make an appointment about 3 days after you have had your tests done to discuss the results with your Doctor. Please make sure we have your current phone number and address when booking appointment.

 

Telephone calls: Our Doctors are unable to take phone calls while attending to patients. If you require to speak to your Doctor, our staff will take a message and the Doctor will call you back as soon as possible.

We advice you call the ambulance 000 for any life-threatening emergencies instead of ringing the practice.

 

Afterhours Service:For general advice about your health or non-emergency symptoms you experience during afterhour period when the medical centre is closed, please call the National home Doctor Service on 13 74 25 or 03) 9429 5677.

For emergency or life threatening symptoms, please call 000 for an ambulance.
